Stomach pain is one of the most common complaints—and in many cases, it’s harmless and goes away on its own. But not all abdominal pain is created equal. Some types of stomach pain signal a serious underlying condition that needs immediate attention.

Knowing which red flags to watch for can help you decide whether to wait it out, head to urgent care, or go straight to the ER.

Stomach Pains You Should Never Ignore

Here are the types of stomach pain that should prompt you to seek medical care—either urgently or immediately:

1. Sudden, Sharp Pain in the Lower Right Abdomen

Possible cause: Appendicitis
This pain often starts around the belly button and moves to the lower right. It may worsen with movement, coughing, or pressure. If untreated, a ruptured appendix can lead to life-threatening complications. Go to the ER immediately.

2. Severe Pain After Eating Fatty Meals

Possible cause: Gallstones
Pain often occurs in the upper right abdomen or under the right ribs, possibly radiating to the back or shoulder. It may be accompanied by nausea or vomiting. Gallstones can block bile flow and require surgical evaluation.

3. Cramping with Bloody or Black Stools

Possible cause: Intestinal bleeding or infection
Blood in your stool is never normal. Bright red blood may suggest a lower digestive issue, while black or tarry stools point to upper GI bleeding. Either warrants immediate evaluation.

4. Burning Stomach Pain That Worsens on an Empty Stomach

Possible cause: Stomach ulcer or H. pylori infection
Ulcer pain often comes and goes, and is usually felt in the upper middle abdomen. Left untreated, ulcers can bleed or cause perforation. Urgent care can often test for H. pylori and start treatment.

When to Go to Urgent Care vs. ER

  • Go to urgent care if:
    • Your pain is mild to moderate but persistent
    • You have nausea, bloating, or discomfort without vomiting or fever
    • You suspect food poisoning, mild gastritis, or early signs of a stomach bug
  • Go to the ER if:
    • The pain is sudden, sharp, or unbearable
    • You have fever, vomiting, or blood in stool
    • You experience fainting, shortness of breath, or chest pain

Urgent care providers can order tests, prescribe medication, and refer you to a specialist if needed. They’re often your best starting point for non-emergency but concerning abdominal issues.
